Mesna is a generic prescription drug used to prevent bleeding and swelling of the urinary bladder resulting treatment with ifosfamide (Ifex) or cyclophosphamide (Cytoxan), cancer chemotherapy drugs.
The body produces acrolein and nitric oxide when it metabolizes (breaks down) ifosfamide and cyclophosphamide. Acrolein concentrates in the bladder and kills the cells in the tissue lining the inside of the bladder, producing bleeding and sores along the inside of the bladder (hemorrhagic cystitis).
When the body metabolizes mesna, one of the substances produced attaches to acrolein and nitric oxide in the bladder and both are removed from the body in urine.
Mesna is administered as three injections or one injection and two Mesnex tablets on the day that cyclophosphamide or ifosfamide is administered. The first is given when the chemotherapy drug is administered, the second a few hours later, and the third a few hours after that.
Most people will experience site reactions to the injection, such as pain, redness, or irritation. In general, the common side effects of Mesna include, but are not limited to, nausea, vomiting, constipation, low white blood cell counts, fatigue, fever, loss of appetite, low platelet levels, weakness, abdominal pain, hair loss, upset stomach, chest pain, and headache.
Serious side effects include low blood pressure, anemia, and low potassium.
Allergic reactions are rare, but can be severe and include anaphylaxis, a dangerous drop in blood pressure, and severe skin reactions. Tell your physician if you notice fever, sore throat, rash, swelling of the face or mouth, racing heartbeats, mouth sores, or trouble breathing.
